He/she has been familiarized with all completed parts of this form, all data is accurate and true and is provided voluntarily. All data provided in this form are collected and processed solely for the purpose of accepting the applicant for study, for the purpose of contacting applicants for study by sending electronic messages for the purpose of communicating information regarding admission to study and study. The summarized data from this application for study may be used by Cosmopolitan Institute for statistical data, for the internal needs of Cosmopolitan Institute in accordance with the purpose set out above and for the purposes of obligations imposed by law. Cosmopolitan Institute declares that it will collect personal data to the extent necessary to fulfill the specified purpose and process them only in accordance with the purpose for which they were collected. Persons working with this data are obliged to maintain confidentiality of personal data, even after the termination of the employment or similar relationship.
